Off the top of my head, the op rod should protrude into the receiver area about 3/4" when fired. It violently pushes the bolt carrier back.
The gas piston moves inside the tube about the same and does NOT go past the holes in the tube. Those hole are to relieve back pressure inside the rear of the tube when the piston compresses the air.

- Bad headspacing
